Please download our PDF file which will give you the opportunity to provide an alternate photo ID (passport or other approved ID) and authorised signature. Your application cannot be processed until we receive this completed form from you.

You can submit your completed application in the following ways:

1. Email a photo of the completed and signed form to info@mytaxback.co.nz
2. Place your ID on a piece of white paper. Write your name and today's date and sign your signature next to your ID. The signature on the ID and the piece of paper must be visible and must be the same. 18+ cards & Kiwi Access Cards do not have a signature... that’s OK!
   • Take a photo of the paper so that you capture the ID and your signature.
   • Email a picture of your ID and signature to info@mytaxback.co.nz
